Anyone can refer to a school nurse team - if we feel unable to help with the referral we will signpost you to another service who may be able to help.
If you are a young person you can self refer, if you are over the age of 13 and are competent to give your own consent. If you are under this age the school nurse may have to get consent from a parent or carer. Call into your School Drop in and the school nurse will try and help.
Parents or carers can refer to the school nurse, just contact your local school nurse team and they will help you.
Professionals - if you would like to refer to the school nurse team, please email a referral form to the school nurse team.
If you are thinking of referring a child/young person into the Children's Continence service, please complete the level 1 check list to ensure it is an appropriate referral into our service. You can find the level one check list and single point of access referral below.
